In order to better serve you, we have developed this web page to assist you in the preparation of your bankruptcy petition. Simply follow the steps below.

IMPORTANT: FOLLOW THE INSTRUCTIONS CAREFULLY AND DO NOT SKIP ANY STEPS
Step 1: Return Fee Agreement
1. Click on the proper link below to print your bankruptcy intake package which includes your Fee Agreement Instructions and and Credit Report Order Form.
2. Print the forms and follow the instructions carefully.
NOTE: PLEASE DO NOT PROCEED WITH ANY FURTHER STEPS UNLESS YOU HAVE MAILED US YOUR RETAINER AND A SIGNED FEE AGREEMENT
Step 2: Confirm your Identity
We use your credit report to obtain a complete listing of your debts as reported by your creditors. In order for us to obtain your credit report, the credit reporting agencies require that you answer a set of security questions to confirm your identity.

Step 3: Complete Bankruptcy Questionnaire
Option 1: Take the Online Questionnaire

Follow these instructions to complete our online Bankruptcy Questionnaire.
Instructions: (Please Read First)
- To begin; enter your social security number below and click on the log in button.
- A secure website will open. Input your personal information; including your assets, income and expenses. Do not list all of your individual belongings, please use categories such as clothing, furniture, jewelry, etc..
- DO NOT ENTER ANY CREDITORS AT THIS TIME; we will get these from your credit report and you will have a chance to add any missing creditors later.
- To save your work and finish at a later time, click on the words "Finish Later" on the top of the page. You may log back in to your questionnaire by returning to this page and re-entering your SSN below. (we suggest that you bookmark this page so that you may return)
- IMPORTANT: When you are finished; click on the words "Finish Now" at the top of the questionnaire.
Option 2: Print out the Questionnaire, Fill it out and Mail it back to us.

Step 4: Wait For Us to Send You Your Draft Petition
Wait for Us.
About a week after you have completed your questionnaire in Step 3, we will prepare a draft bankruptcy petition and mail it to you to review. Please review it carefully and make changes directly on the draft. Follow the instructions on the cover page that we send you and return the documents we request.
Step 5: Take Credit Counseling Course
When instructed, please follow these instructions for completing the online credit counseling.
Pre-Filing Instructions

Be sure to enter our attorney code: MA086
- Note: you are responsible for the cost of this course. ($35 for 1 or 2 filers)
- Remember the password you create, you will need it after your bankruptcy is filed
Step 6: Wait for Your Attorney
Click here to print out and read the "Notice to Individual Bankruptcy Clients".This is required by the Bankruptcy Court- Wait for instructions from your attorney. Once your paperwork is completed and your legal fees are paid in full, your attorney will contact you to schedule a date and time for you to sign your petition.
- If you receive any court notices during this time, please contact your attorney.
Step 7: Post-Filing Instructions
Do not take this course until instructed to do so by your attorney!!!
