Step 1: Return Fee Agreement
1. Click on the proper link below to print your bankruptcy intake package which includes your Fee Agreement Instructions and and Credit Report Order Form.
2. Print the forms and follow the instructions carefully.
NOTE: PLEASE DO NOT PROCEED WITH ANY FURTHER STEPS UNLESS YOU HAVE MAILED US YOUR RETAINER AND A SIGNED FEE AGREEMENT
Step 2: Confirm your Identity
In order for us to obtain your credit report, the credit reporting agencies require that you answer a set of security questions to confirm your identity.

Step 3: Compile Important Documents
Pursuant to the bankruptcy code, we are required to obtain certain documents from you to verify the information you provide us to prepare your bankruptcy petition. These documents will assist you in answering your bankruptcy questionnaire in the next step. Click on the link below and collect the documents that are listed.

Step 4: Complete Bankruptcy Questionnaire
Option 1: Take the Online Questionnaire
(Preferred Method)
Follow these instructions to complete our online Bankruptcy Questionnaire.
Instructions:
- To begin; enter your social security number below and click on the log in button.
- A secure website will open. Input your personal information; including your assets, income and expenses. Do not list all of your individual belongings, please use categories such as clothing, furniture, jewelry, etc..
- DO NOT ENTER ANY CREDITORS AT THIS TIME; we will get these from your credit report and you will have a chance to add any missing creditors later.
- To save your work and finish at a later time, click on the words "Finish Later" on the top of the page. You may log back in to your questionnaire by returning to this page and re-entering your SSN below. (we suggest that you bookmark this page so that you may return)
- IMPORTANT: When you are finished; click on the words "Finish Now" at the top of the questionnaire.
NOTE:
- Do not begin this questionnaire until you have returned your fee agreement and paid your retainer; otherwise your information will be deleted.
- Please enter the information on this form as completely and accurately as possible.
Option 2: Print out the Questionnaire and Mail it back to us.
Step 5: Take Credit Counseling Course
Follow these instructions for completing the online credit counseling.
Pre-Filing Instructions

Be sure to enter our attorney code: MA086
- Note: you are responsible for the cost of this course. ($35 for 1 or 2 filers)
- Remember the password you create, you will need it after your bankruptcy is filed.
Step 6: Read and Wait
Click here to print out and read the "Notice to Individual Bankruptcy Clients".This is required by the Bankruptcy Court- Wait for instructions from your attorney. In a few days, you will receive your draft petition. Review it carefully and follow the instructions that are provided.
Step 7: Post-Filing Instructions
Do not take this course until instructed to do so by your attorney!!!
